Hospice is a specialized care for those individuals who have a terminal diagnosis of six months or less. They choose hospice care when they and their physicians have decided not to pursue curative treatments any longer.


For over 29 years, the professional staff of JourneyCare (Formally Hospice and Palliative Care of Northeastern Illinois) has been taking care of terminally ill patients. We know that patients and their families are facing a difficult time, and we promise to do everything we can to make it as comfortable and peaceful as possible.

When pain is an issue... Palliative care may be the solution. Palliative care is a medical sub-specialty focused on alleviating pain and symptoms at any stage of an illness.

When the illness becomes life-limiting... We provide care and support to individuals and their families during this dif cult time. Our services and care embrace the emotional, social, spiritual and practical aspects of life.

If caring at home becomes too difficult... If illness and/or end-of-life issues become unmanageable, or the care setting requires a change, we offer a home-like environment and medical care around the clock at our Barrington and Woodstock locations.

Heartland Hospice Services

Every patient is different.

Heartland Hospice provides care to patients wherever they call home: in the house they've lived in for years, a skilled nursing center, an assisted living facility or independent living facility.

Every patient is different. With different challenges, different needs, different family and friends. So the care we give each patient is different. Our caregivers work with patients to create individualized care plans and set personal goals. They encourage patients to make the right decisions on their own behalf and help fill their lives with meaningful experiences. Pain and symptoms are managed so patients can be as comfortable as possible. Our social workers and spiritual counselors help patients and families share thoughts, hopes, concerns and dreams.

We take our roles in our patients' lives very seriously. Their dignity must be preserved, and respect always shown to them.

Midwest Palliative & Hospice CareCenter is a nonprofit, community-based healthcare organization providing hospice care, palliative care and grief support in north and northwest Chicago, the north suburbs and Lake County, Illinois.

Hospice care provides end-of-life medical and supportive care where you live. At the center of hospice care is the belief that each of us has the opportunity and right to live with dignity, free of pain and other distressing symptoms, connect with those that we love in meaningful ways and know that our loved ones will be supported by the professional hospice care team.

Palliative care is consultative medical care by an interdisciplinary team that promotes quality of life and relieves suffering. Palliative care can occur simultaneously with treatments that have cure and recovery as their goal.

Grief counseling and grief support groups are available to adults, teens and children. Grief support groups provide a nonjudgmental and confidential safe haven for working through grief.
